The RN Case Manager puts together the patient’s care and services given by the hospice team. Also, the hospice nurse works on symptom control and pain management.
Hospice Certified Nurse Aid
The Hospice Certified Nurse Aide gives personal care under the supervision of the RN Case Manager. In addition, help with bathing and grooming.
Social Worker
The Social Worker gives psychological and emotional assessment of the needs of the patient and family. Information and community resources are also given.
Bereavement
Many programs give Bereavement services. To sum up, it gives support, guidance, and education.
Chaplain
Chaplains are available to help our patients and family members connect to their faith tradition. Certainly, Spiritual Care resources can help people return a sense of balance into their lives.
