Our Home Health team is ready to listen to your concerns, discuss your options, answer all your
questions, arrange for services and work with your insurance provider. Also, we offer coordinated
interdisciplinary services to address any home care needs and work in partnership with referring
physicians and other providers to deliver coordinated, compassionate, professional care from the
simplest to the most complex cases. Certainly, home health services are available 24 hours a day, seven days a
week.

How to get referred to Home Health:

doctor’s order is required for home health services.  The physician must certify that you need home health services.
 

Most insurances including Medicare require that a doctor or a certain health care professionals who work with a doctor (like a nurse practitioner), must document that they have had a face-to-face encounter with you (like an appointment with your primary care doctor) within required time frames and that the encounter/visit was related to the reason you need home health care.

Some insurances and Medicare require that you are homebound.  To be homebound means: You have trouble leaving your home without using an assistive device (walker, cane, wheelchair) or help from another person because of an illness or injury, or if it isn’t recommended because of your condition.  You’re normally unable to leave your home, but if you do it requires a major effort.
